Summary

Position Summary

We have an exciting opportunity for an experienced Community Health Worker to join our growing Population Health Management (PHM) team! The PHM department provides complex care management services to patients across the MaineHealth Medical Group (MHMG) and is embedded within the primary care setting. As part of an interdisciplinary team, the CHW will work directly with patients in our complex care management program to identify and address social determinants of health (SDOH) related barriers to care such as food or housing insecurity, transportation, care affordability and health literacy.

Interactions with patients will include telephone, virtual visits, face to face in the practice, or in the patient's home or community setting - whichever option best meets the needs of the population being served.

The role is not expected to perform hands-on or direct patient care, nor are they expected to transport patients in a vehicle.

We are looking for someone who enjoys helping others, being part of an evolving team, helping to integrate clinical programs and community resources into the practice workflows with the intent of optimizing patient care, improving patient and provider experience and enhancing the quality of care provided to our patients.

Required Minimum Knowledge, Skills, and Abilities (KSAs)
  1. Education: High school diploma or equivalent.
  2. License/Certifications: Candidates must possess a valid Maine driver's license, maintain a clean driving record and have a personal vehicle available for work use.
  3. Experience: Must have lived experience in the community being served, preferred. If relevant, bilingual in English and any languages specific to the communities being served.
  4. Has a close understanding of the community being served with good knowledge of the resources of their community.
  5. Experience in outreach, customer service, social service, or related field preferred.
  6. Experience in translation is preferred if relevant to the community being served.
  7. Excellent oral and written communication skills.
  8. Fluency (verbal and written) in specified language as needed.
  9. Ability to carry out written and oral instructions.
  10. Ability to exercise judgement in the application of professional services.
  11. Comfortable with home visits.
  12. Comfortable with Microsoft Office suite.
