Position Summary

The Health Education Navigator will work jointly with the Medical, Social Services, and Education Departments at Mary's Center through the Healthy Lifestyle program. This is a grant-funded position that will provide education, support, and care coordination in efforts to prevent disease and promote well-being for Mary's Center participants. The candidate will provide health education through one-on-one and group education and outreach sessions with adult, adolescent, and pediatric populations using needs assessments, care plans, and self-management goal setting. The candidate will also provide participants with navigation to chronic care, treatment services, and other support services to promote positive health outcomes.

Reportability

This position will report directly to the Community Health Education Director.

Essential Duties and Responsibilities

To perform this job successfully, an individual must be able to perform each essential duty satisfactorily.

  • Deliver health education on a variety of health topics.
  • Review research on health education topics to provide participants with relevant information related to health.
  • Focus on disease prevention and well-being promotion in the continuum of the life cycle.
  • Provide individualized, one-on-one health education to participants.
  • Conduct outreach, informal educational chats, and workshops speaking with groups of individuals and families.
  • Understand and utilize best practices and research-informed techniques and strategies in health education and navigation.
  • Distribute linguistically and culturally appropriate educational literature/information.
  • Provide care coordination and healthcare navigation support to participants.
  • Communicate with care team members to facilitate care for participants.
  • Schedule follow-up visits and referrals to the team of care.
  • Assist in the data management of patient preventive wellness and health maintenance activities.
  • Maintain documentation of encounters and provide reports in reference to programmatic activities as requested by Mary's Center staff and funders.
  • Attend training and conferences as needed for professional development.
  • Establish timelines with supervisor on specified goals and day-to-day tasks needed for the program. To be evaluated at 90-day and 1-year performance reviews.
  • Perform other duties as assigned.

Qualifications

Knowledge, Skills & Abilities -

  • Minimum of a college degree focusing in health and education or related field.
  • Must possess experience working in a social services and/or health care agency, having worked directly with clients, and program/s coordination.
  • Skills necessary to coordinate solution-focused activities with multiple agencies, and ability to provide advocacy is a plus.
  • Must have a thorough understanding and working knowledge of patient confidentiality issues and HIPAA compliance/regulations.

Language Requirement - Must have the ability to communicate effectively in English and Spanish in second language used by the target audience.

Physical Demands - Regularly required to sit; frequently required to reach with hands and arms, walk, stoop, kneel, crouch, talk or hear; must be able to lift objects up to twenty-five (25) pounds.

Work Environment - Must be available to work some weekends, if needed. Mostly in a typical office and/or home setting with quiet to moderate noise level. May be exposed to extreme cold, heat, and humidity due to outside weather conditions. Opportunity to work a hybrid schedule - remotely & onsite, as needed.
