1. Accountable as nutrition liaison/expert in Weight management responding to needs and initiatives for nutrition care process improvement.Coordinates, promotes, and provides outpatient nutrition services to the weight management population and provides group and individual nutrition counseling to individuals pursuing weight loss.

2. Assesses the nutrition needs of weight management patients by completing a nutrition assessment, identifying nutrition risk, include diet education and an individualized nutrition care plan within the EMR.

3. Complete and document calorie counts/dietary recall/assessments and follow-ups accurately according to standards of care; including but not limited to: appropriateness of diet to diagnosis, adequacy of current diet, assessment of laboratory values, drug-nutrient interactions, medical history, current nutritional needs, nutrition education needs, care plan development and writing orders in the electronic medical record when nutrition intervention is needed for a patient.Document summary of and communicate results to medical staff and patient.(if applicable)

a.The Registered and Licensed Dietitians (RD/LD) at WVU Medicine have been granted Order Writing Privileges which have been approved and established by the WVU Medicine Medical Executive Committee and the WVU Medicine Board of Directors.These privileges may be utilized whenever the registered dietitian is involved in patient care and allows the RD/LD to order medical Nutrition Therapies within their scope of practice as defined by the Academy of Nutrition and Dietetics and West Virginia Board of Licensed Dietitians (WBOLD).

b.The RD is considered qualified with current registration from the Commission on Dietetic Registration, current

licensure through the WVBOLD, and clinical competency as verified by the Clinical Nutrition Manager initially

and through the annual performance appraisal process.(See RD order writing privileges policy for specific

guidelines)

4. Participates in all appropriate weight management team meetings and support groups and will participate in monitoring QI metrics to plan and implement performance improvement strategies including fiscal, clinical, and patient satisfaction data to improve program operations

5. Collaborating with all members of the multidisciplinary team to facilitate the care of patients, and educating the team about clinical pathways and protocols

6. Effectively communicate with nursing, medical and other hospital and clinic staff to ensure high quality nutritional care.Attends care plan conferences or any other meeting/conference that physicians request dietitian attendance or those meetings/conferences required to maintain accreditation of hospital programs or foster partnership with other facilities.

7. Helping with implementation of patient info sessions and medical weight loss classes including patient flow and helping to achieve compliance with required documentation.

8. Managing and updating program's website and social media.

9. Monitoring patient progress to eliminate any barriers to high quality patient-focused plans of care including compliance such as food logging when applicable, weigh-ins.

10. Maintaining a working knowledge of the available services for weight loss around the state, to assist patients in receiving services in convenient locations

11. Collaborating with available social services to access and mobilize appropriate resources including financial assistance, and family/community resources to meet identified emotional, social, and financial needs

12. Liaison with surgical weight loss program

13. Coordinating WV weight management CME workshops at WVU

14. Coordinating telemedicine model such as ECHO program and less formal internal meetings regarding difficult cases

15. Traveling to system hospitals to provide education and training

16. Helping with implementation of educational initiatives that create obesity medicine workforce for state:resident and med student rotations/tracks, obesity medicine fellowship, APP training, primary care partnerships

17. Partnering with stakeholders around the state for outreach and participating in advocacy for obesity treatment (improved insurance coverage for medications and surgery, eliminate prior auths, for example)

18. Review and update clinical rotation assignments annually to reflect changes in practice and feedback from previous interns.

*Monitor interns, making certain that they are functioning to standards before leaving unsupervised. Review and

co-sign all intern documentation in the medical record

*Correct assignments and evaluate progress within 2 weeks of the end of the rotation

*Develop and teach classes regarding area of specialization to the interns (i.e. Pediatric nutrition, formulas, etc.)

*Provide a caring and supportive learning environment; provide a high degree of interaction and explanation.

*Attend intern case study presentations, question interns, evaluate presentation.

19. Develop and present in-services/lectures to weight management support groups, other health care personnel, and students if needed.

20. In-service Dietitians on information from any educational meetings attended.

SKILLS AND ABILITIES:

1. Requires use of EPIC computer system along with being skilled in computer programs such as Word, Excel and Power Point.

2. Experience with Nutrition analysis software preferred.

3. Able to speak in front of a group.Professional appearance and demeanor. Ability to utilize strong organizational and time management skills to meet established goals and objectives.Good verbal and written communication skills.

Additional Job Description:

The Surgery/Bariatrics Clinical Dietitian works with the medical team to provide optimal medical nutrition therapy to patients in the Bariatric clinic and upon post-surgery follow-up in the hospital. Works with members of the department to improve quality care to patients. Coordinates care with inpatient dietitians.
