Job Description
POSITION SUMMARY: The Patient Navigator is a member of the multi-disciplinary population health team that assumes responsibility and accountability for the management of resources through interdisciplinary collaboration to achieve optimal patient outcomes.
Minimum Qualifications:
-Associate's degree in Healthcare Management, Public Health, or other healthcare related field
-OR Bachelor's degree with one year of Customer Service experience
