Summary

Excellent MSW opportunity, 26 Hours/week, Day shift, no weekends, or Holidays .

Position Highlights:

  • Flexible and autonomous schedule
  • Work with an engaged, clinically excellent, and supportive team at CHANS Hospice.
  • Case Manage a set number of cases in beautiful Mid Coast Maine. Territory will mostly be in Bath, Brunswick, and surrounding coastal towns.
  • Have the ability to work one on one with patients and families.
  • Ability to give your full, undivided attention to each patient.

Position Summary

The CHANS Hospice Social Worker helps clients, and their families navigate planning for end-of-life care; understand their treatment plan and be vocal about their needs; manage the stresses of debilitating physical illnesses, including emotional, familial and financial; overcome crisis situations; and connect to other support services. This position reports to the Clinical Manager

 

  • Required Minimum Knowledge, Skills, and Abilities (KSAs)
    1. Education: Bachelor’s degree in Social Work.
    2. License/Certifications: Current Maine licensure as a LSW.
    3. Experience: Office setting experience.
